MASH Application Process and Required Documents

STEP 1 - Reservation Request Form

In this initial step the applicant can reserve MASH funds. The project does not have to be complete, or even started in order to secure a reservation. Upon successful submission of the following documents the Program Administrator will submit a reservation notice detailing the amount reserved.

All submission information can be found in the Reservation Request Form. Reservations are good for 18 Months.

  1. Completed Reservation Request Form and Completed Contract with Original Signatures (See Forms Below)

  2. Proof of Electric Utility Service for Site (Most Recent Electric Bill) 

  3. Electrical System Sizing Documentation (If Applicable)

  4. Documentation of CPUC Code 2852 Eligibility (Proof of Low-Income Status)

  5. Documentation of an Energy Efficiency Audit or Title 24 Documentation or Other Exemption (see link to energy audit tools below). 

  6. Copy of Signed Energy Efficiency Disclosure Form (Beginiing July, 1 2009)

  7. Printout of EPBB Tool Calculation (www.csi-epbb.com)

  8. Completed Form "Authorization To: Receive Customer Information or Act on a Customer's Behalf"

STEP 2 - Proof of Project Milestone


In this step the Program Administrator requires an additional set of documents that demonstrate the project is moving forward. 

  1. Completed Proof of Project Milestone Checklist

  2. Copy of Executed Contract for System Purchase and Installation

  3. Copy of Executed Alternative System Ownership Agreement (If System Owner is Different than Host Customer)

  4. Revised EPBB Calculation Printout (If Applicable, www.csi-epbb.com)

  5. Copy of Request for Proposal (RFP) or Solicitation (If Applicable)

STEP 3 - Incentive Claim Form

The following documentation must be submitted after the solar system is installed and interconnected.

  1. Completed Incentive Claim Form with Original Signatures

  2. Copy of Executed PMRS Contract or PMRS Cost Cap Exemption Documentation

  3. Revised EPBB Calculation Printout (if Applicable)

  4. Final Project Cost Breakdown Worksheet

  5. Final Project Cost Affidavit

  6. Documentation of Tenant/Common Load Allocation
