Reservation Request
Required Documents for All Projects:
- Completed Reservation Request Form 2012 with signatures
- Copy of Executed Contract or Agreement for Installation (includes warranty language documentation)
- Equipment Specifications
- Proof of Utility Service: submit a copy of a recent electric or gas utility bill indicating the account number, meter number, site address and host customer name.
- 12-Month Electric Load Documentation: submit a copy of the previous 12 months of energy consumption including maximum demand and/or kWh consumption.
- Energy Efficiency Audit: the audit must have been performed during the past five years and must identify feasibility and payback periods of energy-efficiency measures.
- Proof of Adequate Renewable Fuel or Waste Energy Resource (applies to all technologies except AES if operating on a renewable fuel or waste energy): submit an engineering survey or study confirming the renewable fuel and the generating system's average capacity.
- Proposed monitoring plan (for projects 30kW and larger)
- Documentation verifying non-profit entity status (if applicable)
- Application Fee: equal to 1% of the requested incentive amount
Additional Documents for Conventional CHP and Fuel Cells:
- Minimum Operating Efficiency Calculations (if applicable):
- Waste Heat Utilization Documentation OR
- Minimum Electric Efficiency Calculation
- Copy of Completed Air Pollution Permit Application (if applicable)
- Fuel Cleanup Equipment Purchase Order (On-site, renewable fuel projects only. Nominated biogas projects exempt)
- Renewable Fuel Affidavit (On-site, renewable fuel projects only.)
- Waste Gas Fuel Affidavit (waste gas projects only)
Additional Documents for Conventional CHP:
- Proof of Power Factor Eligibility
- Proof of NOx Emissions Qualifications (Non-renewable fuel projects only):
- Minimum 60% System Efficiency Calculation
- Emissions Credits Calculation (if applicable)
- Proof of Adequate Waste Gas Fuel (Waste-gas fuel projects only): submit an engineering survey or study confirming that there is adequate on-site waste gas fuel.

- Copy of Executed Renewable Fuel Contract
- Forecasted Fuel Consumption

- Substantiation of Load (New Construction/Expanded Load Only)
- Revised Sizing Calculations (if applicable)
- Performance Data Provider Documentation (for projects 30kW and larger)
- Final Monitoring Schematic (for projects 30kW and larger)
- Payment reassignment form (if applicable)
Additional Documents for Conventional CHP and Fuel Cells:
- Revised Minimum Operating Efficiency Calculations (if applicable):
- Waste Heat Utilization Documentation OR
- Minimum Electric Efficiency Calculation
- Final Air Permit Documentation (if applicable) (nominated biogas projects exempt)
- Final Fuel Cleanup Skid Cost Documentation (on-site renewable fuel only, nominated biogas projects exempt)
- Substantiation of Renewable Fuel Resource (on-site renewable fuel only)
- Maintenance coordination letter (for CHP projects over 200 kW only)
